緑内障を合併したマルケサニ症候群に眼内レンズ挿入術を施行した1例

Intraocular lens implantation in a case of Marchesani syndrome with secondary glaucoma

　YAGレーザーによる前嚢切開を併用した超音波水晶体乳化吸引術（PEA）と眼内レンズ（IOL）挿入術を施行し，著明な眼圧低下が得られたマルケサニ症候群の1例を経験した。症例は40歳女性。初診時左視野狭窄を訴えたが疼痛はなく，眼圧は両眼とも50mmHg以上で，浅前房と狭隅角があったが虹彩前癒着はなかった。散瞳後小球状水晶体，乳頭には緑内障性陥凹を認めた。薬物療法に抵抗したため，両眼にPEAとI0L挿入術を行った。術後両眼とも正常眼圧となり，高度近視も解消され，現在はI0Lの固定も良好である。薬物療法に抵抗する緑内障を合併したマルケサニ症候群に対しては，I0L挿入術を施行しても良いと思われた。

A 40-year-old female presented with visual field loss in her left eye. She manifested shallow anterior chamber, narrow angle, microspherophakia and glaucomatous disc cupping in both eyes. Peripheral anterior synechia was absent. The intraocular pressure was 50mmHg in both eyes and did not respond to medical therapy. Both eyes were treated by anterior capsultomy with YAG laser, phacoemulsification-aspiration and intraocular lens implantation. The surgery was followed by well-controlled intraocular pressure and improved refraction. The present method is advocated as a preferred surgery for Marchesani syndrome with secondary glaucoma.
