要約　目的：球面または非球面眼内レンズ（IOL）を挿入したのちの波面収差とコントラスト感度の変化の報告。対象と方法：16例32眼を対象とし，一眼に球面，他眼に非球面IOLを挿入した。男性5例，女性11例で，平均年齢は76歳であった。術後約1か月後と約4年後に，波面収差とコントラスト感度を測定した。結果：術後早期の計測では，解析領域5mmでの全高次収差，球面収差，コマ収差が球面IOLよりも非球面IOLで有意に小さく，術後長期では，球面収差が球面IOLよりも非球面IOLで有意に小さかった。結論：術後の球面収差は，短期と長期ともに非球面IOLが球面IOLよりも小さい。

Abstract.　Purpose：To report the long-term changes in aberration and contrast sensitivity after implantation of spheric and aspheric intraocular lenses（IOLs）. Cases and Method：This prospective study was made on 32 eyes of 16 patients who received cataract surgery. Each patient was implanted with spherical IOL in one eye and aspherical IOL in the fellow eye. The series comprised 5 males and 11 females. The age averaged 76 years. They were examined for aberration and constrast sensitivity one month and 4 years after surgery. Results：Total higher-order,spherical and coma aberrations were significantly smaller in eyes implanted with aspherical than spherical IOL one month after surgery. Spherical aberrations were smaller in eyes implanted with aspherical than spherical IOL 4 years after surgery. Conclusion：Spherical aberrations are smaller in eyes implanted with aspherical than spherical IOL after surgery on a long-term basis.
