A 51-year-old male presented with blurring and central scotoma in his right eye. Funduscopy showed grayish opacity of 2 disc diameters across in the macula. The visual acuity was 0.3. Fluor-escein angiography showed irregular hyperfluor-escence in the opaque area from the early phase and tissue staining towards the late phase. The retinal veins were slightly hyperpermeable to fluorescein. The left eye was unaffected throughout. He was diagnosed as acute syphilis because of seropositive findings. The visual acuity improved to 1.2 after 4 weeks of systemic ampicillin 2 grams daily.
