種々の保存療法にもかかわらず,角膜上皮びらんをくりかえした難治性の糖尿病性角膜症の1例に,角膜表層穿刺の原理にもとづいてNd:YAGレーザーを照射し治癒せしめた。レーザー照射後3日で角膜上皮は修復された。角膜内皮細胞への影響はほとんどなく,その後の再発も認められなかった。本法は角膜への微細な照射が可能であり,照射前後の処置も特に必要とせず,点眼麻酔下で安全に施行し得る。
糖尿病以外の難治性の角膜上皮接着障害にも応用可能と考える。
A 61-year-old diabetic male underwent extra-capsular cataract extraction in his left eye. Corneal erosion developed on the day after surgery and healed spontaneously 2 weeks later. Secondary intraocular lens implantation was performed 5 weeks later. Transient corneal erosion developed immediately after surgery. Transscleral retinal cryocoagulation for the right eye was performed 14 months later for vitreous hemorrhage. Large corneal erosion developed the next day. Nd: YAG laser was applied to coagulate the affected corneal stroma. The erosion showed marked improvement after 3 days. Specular microscopy showed intact corneal endothelial cells. Nd: YAG laser treatment may be considered as an effective method for severe corneal erosions in diabetic eyes due to defective epithelial adhesion.
