緑内障術後前房消失例の治療経験—上脈絡膜腔貯溜液排除法

Treatment of flat anterior chamber following filtering glaucoma surgery by drainage of suprachoroidal fluid

　緑内障濾過手術後の難治性脈絡膜剥離の症例に前房形成を目的として昇術を施行した。方法は強膜弁を作成して，1／2層の強膜弁下に強膜切開を行い脈絡膜に達する切開をした。上脈絡膜腔液を排泄した後，切開創縁に牽引糸をかけ，創が閉鎖しにくいようにした。術後に前房は形成され，眼圧のコントロールも良好であった。上脈絡膜腔貯溜液を伴う回復傾向のない前房形成不全例には強膜弁下の強膜切開創に糸を留置する方法が有効であると思われた。

We performed trabeculectomy in an eye with chronic closed angle glaucoma. The anterior cham-ber became flat on day 3 of surgery associated with ocular hypotension. We treated this condition with a newly designed surgical technique of anterior chamber reconstruction.

We incised the scleral and kept this wound open with a single-armed suture between the edge of scleral incision and dissected scleral edge. The suprachoroidal fluid continuously leaked through this incision. The anterior chamber was then refor-med by injecting hyaluronic acid and air. This manipulation resulted in immediate reformation of anterior chamber. The intraocular pressure remained under 10 mmHg without medication.
