両眼開放による白内障手術—小型DVD液晶カラーテレビの応用

Cataract surgery with both eyes open.:Use of a portable color television for fixation by the fellow eye

　白内障手術中の眼位を安定させる目的で，両眼を開放し，僚眼で小型DVD液晶カラーテレビ画面を固視させた。手術直前にテレビ画面を観賞させ、手術開始後にはこれを視標として用いた。この方法で61例61眼に白内障手術を行った。僚眼による画面固視により，55例（90％）で手術眼の眼位が安定し，手術操作が円滑に行うことができた。術前のテレビ画面観賞では，高血圧のない患者で血圧が安定した。この方法は，手術眼の眼位を安定させ，患者の不安と緊張を緩和し，安全に手術を行うために有効であった。

We performed cataract surgery on 61 eyes of 61 patients with both eyes open. Before start of sur-gery, the patient was shown a program on a portable color television (TV) placed on a stand of surgical table. After start of surgery, the TV display terminal was used as fixation target by the fellow eye. With this proce-dure, the operated eye remained well-fixated during surgery in 55 cases (90%). Looking at the TV program before surgery induced stabilization of blood pressure in nonhypertensive patients. The present method was effective in stabilizing the eye position of operated eye, in eliminating anxiety or stress, and in the safety of cataract surgery.
