前部硝子体手術を併用したphacolytic glaucomaの水晶体摘出術

Phacolytic glaucoma. Inflammatory extension to the vitreous and surgical approach

　3例4眼のphacolytic glaucomaについて白内障手術を施行したが，4眼とも硝子体圧がつよい傾向にあった。1眼はあらかじめ前部硝子体手術を行ったところ，良好な結果をおさめた。

　硝子体手術時に採取した硝子体液を免疫電気泳動法にて分析した結果，アルブミン，トランスフェリン，IgGの明らかな増加がみとめられたことから，硝子体への炎症の波及が示唆された。

　手術時，硝子体圧がたかかった原因については，手術までの炎症に対する治療が関係しているものと思われ，直接的には硝子体の炎症との関連性がつよくうかがわれた。

　Phacolytic glaucomaでは，早期に白内障手術を施行する場合，強力な消炎療法を行い，硝子体手術の準備をしておくか，もしくは，あらかじめ前部硝子体の切除，吸引を行っておくことは，硝子体の分析結果からも理にかなった治療法かと思われる。

We performed cataract surgery on 4 eyes in 3 cases with phacolytic glaucoma. Vitreous pressure was high in 4 eyes during surgery. Lens extraction in combination with anterior vitrectomy was un-eventfully performed in one eye.

Immunoelectrophoresis of the vitreous humor showed a marked increase in albumin, transferrin and IgG, suggesting inflammatory extension to the vitreous.

Increased vitreous pressure seemed to be related to both vitreous inflammation and anti-inflamma-tory therapy.
