　1994年6月〜1995年6月に超音波白内障手術を行った115例118眼の老人性白内障患者に対して，超音波乳化吸引法による手術侵襲を少なくする目的で，粘弾性物質を用いた水晶体核切開一分割を試みた。前嚢切開創から挿入した佐藤角膜刀により水晶体核の2／3の深さに達するように水晶体を線状に切開し，粘弾性物質を注入して水晶体核を分割した。水晶体核の硬さに応じて，平行切開，十字切開を行った。水晶体核切開一分割法は，核分割法やphaco chop法などの超音波手法にも応用可能であり，水晶体核の超音波処理が短縮され，安全に施行しえた。水晶体核切開—分割法は，現状では唯一の簡易な補助的水晶体核分離法であり，術者の心理的負担が軽減された。

We used an incision and fraction of the lens nucleus during phacoemulsification and aspiration surgery in 118 cataract eyes. Following capsulectomy, Sato corneal knife was inserted into the anterior chamber through a small corneoscleral incision to create a linear incision of the lens nucleus. Viscous solution of hyaluronic acid was then injected into the cite of incision to fragment the lens nucleus. The ensuing phacoemulsification was facilitated by this incisive and fraction method. This method appeared to be of value in protecting the corneal endothelium from the damage of ultrasound and in enhancing the safety of phacoemulsification-aspiration procedure.
