高粘弾性ピアルロン酸ナトリウム製剤を用いた全層角膜移植術

Penetrating keratoplasty aided by high-viscosity sodium hyaluronate

　高粘弾性ピアルロン酸ナトリウム製剤，Healon^®GVを用いた全層角膜移植術13例に対し，その手術時の有用性，安全性について検討した。Healon^®GVはその高い粘性のため，手術中常に前房を深く保持し，圧迫などによって流出しにくかった。また虹彩の癒着に対して，強い剥離効果を発揮した。アセタゾラミド内服を併用することにより，術後翌日の眼圧は12.3±6.9mmHgで，術前との有意差はなかった。術後7か月の角膜内皮細胞密度は2.063.6±307.5／mm²，細胞減少率は18.3±14.5％で，過去の報告に比べて同等であった。本製剤は全層角膜移植術において有用かつ安全と思われた。

We used high-viscosity hyaluronate at the con-centration of 1.4% as adjunct to penetrating ker-atoplasty in 13 cases. It was possible to release posterior synechia when injected under pressure. Only four preplaced sutures were enough to main-tain the anterior chamber during suturing of the graft. The endothelial cells 7 months after surgery averaged 2063.6±307. 5/mm² with the mean cell loss rate of 18.3±14.5%. These findings show the effectiveness and safety of high-viscosity hyaluronate as adjunct to penetrat-ing keratoplasty.
