結膜および眼窩にみられたextramedullary plasmacytomaの1症例

A case of extramedullary plasmacytoma of conjunctiva and orbit

　結膜および眼窩にみられたextramedul—lary plasmacytomaの1例を経験した。症例は78歳の女性。左下眼瞼球結膜に弾性硬で可動性に乏しい腫瘍を認め，CT検査で左下眼瞼結膜下より眼窩へ進展する腫瘍陰影を認めた。両初期白内障以外，眼科学的には特に異常所見はなかった。血液および尿検査にても異常を認めなかった。腫瘍摘出，生検術を行い，腫瘍は免疫染色によりIgGχチェーンに対し陽性を示すplasmacytomaであった。骨髄検索を含め，全身検索にてほかに異常所見を認めなかったため，本症例をextramed-ullary plasmacytomaと診断した。

A 78-year-old woman presented with tumorous lesion in the conjunctiva and orbit in her left eye. Surgically extirpated mass showed plasma cell tumors. Immunoperoxidase staining of the tissue was positive for IGG-kappa. Immunoelectropho-resis of the serum and urine gave negative results. Systemic examinations showed no tumors. Bone marrow aspiration was normal. We diagnosed this case as extramedullary plasmacytoma of con-junctiva and orbit.
