結膜，眼瞼および眼窩における悪性リンパ腫の臨床病理学的検討

Clinicopathological features of malignant lymphoma in the conjunctiva, eyelid or orbit

　眼科領域に原発し，免疫組織学的に悪性リンパ腫と診断された16症例を臨床病理学的に検討した。

　LSG分類によると，びまん性大細胞型が3例，中細胞型が8例，小細胞型が3例，混合型が1例，リンパ芽球型が1例で，従来の報告とは異なる分布を示した。モノクローナル抗体MT-1，MB-1，ポリクローナル抗体抗IgG, IgA, IgM，κ，λを用いて免疫組織化学染色を行った。1例でMT-1陽性，14例でMB-1陽性であった。中細胞型3例と小細胞型2例で胞体内免疫グロブリンが陽性で単クローン性を示した。B細胞性が大部分を占め，単クローン性の増殖であるという眼科領域の悪性リンパ腫の特徴がみられた。

　びまん性大細胞型は転移しやすく，予後不良で，小細胞型は転移が少なく予後良好であるという従来の報告に一致した結果が得られた。

We evaluated 16 cases of primary malignant lymphoma in the conjunctiva, eyelid or orbit. The diagnosis was confirmed immunohistologically. The series comprised diffuse large cell type 3 cases, diffuse medium cell type 8, diffuse small cell type 3, mixed type 1 and lymphoblastic type 1. Immunohis-tochemical studies with monoclonal antibodies, MB-1 was positive in 14 cases and MT-1 in 1. Intracytoplasmic immunoglobulins were positive in 3 cases of diffuse medium-sized cell type and 2 cases of small cell type. The lymphomas in the present series were dominantly derived from B cells and underwent monoclonal proliferation. Diffuse large cell type tumor showed a tendency to meta-stasis with poor prognosis. Small cell type tumors were characterized by infrequent metastasis with fair prognosis.
