眼窩内plasmacytomaの1例

Plasmacytoma of the orbit: A case report

緒　言

　plasmacytomaは網内系とくに骨髄に由来する腫瘍であり，本来多発性でmultiple myelomaとして知られている。時に単発性（solitary），または骨髄外性（extramedullary）の発生もあるが，これらは一般にいずれ多発性となる初期像にすぎないとか，あるいは多発性のものの異型（variant）であると考えられている。そしてその発生頻度も稀で，extramedullaryとしては呼吸器，鼻咽腔のものが大部分であり，眼窩内のplasmacytomaは大変珍しく本邦での報告例はみられない。われわれは最近この眼窩内に原発したplasmacytomaを経験したので報告する。

Among the neoplasms of the reticuloendothelial system, extramedullary solitary plasmacytoma originating in the orbit is a very rare manifestation. We reported a 50-year old man with severe left exophthalmos which progressively protruded and finally resulted in the loss of vision over a 2-year-period (Fig. 1). The orbital tumor was successfully extirpated (Fig. 5), and plasmacytoma was con-firmed light- and electron-microscopically (Fig. 6, 7, 8, 9). There were no other lesions suggesting the involvement of plasmacytoma through whole-body bone survey with X-ray, RI scan and bone marrow biopsy.
