A 50-year-old male presented with sudden visual loss in his right eye. He had been treated for diabetes mellitus with good control for the past 5 years. The optic disc appeared pale and edematous. Fluorescein angiography showed filling defect in the disc. We diagnosed him as anterior ischemic optic neuropathy (AION). The visual acuity failedto improve despite systemic corticosteroid therapy. The optic disc in the left eye was swollen when seen by us 2 months after onset. The superficial disc capillaries were dilated with splinter hemorrhage and cotton-wool patches around the disc. Fluores-cein angiography showed dye leakage from the dilated superficial disc capillaries. The disc swell-ing resolved spontaneously 3 months later.

We interpreted the disc swelling in his left eye as diabetic papillopathy. Obstruction of the superficial disc capillaries seemed to be the causative factor.
