36歳女性の片眼性の角膜脂肪変性症に対し表層角膜移植術を施行した。その病理組織所見は，光学顕微鏡において，角膜実質内は膠原線維の走行が乱れ，細胞が集積し，その細胞質内に脂肪染色により中性脂肪を証明した。角膜実質内に血管侵入も認めた。電子顕微鏡において，組織球が多数の脂肪球を細胞内に取り込んでいた。本症例は，眼疾患の既往はないが，無症候性の角膜の炎症があって血管侵入が起こり，続発性に角膜脂肪変性症が発生したと推察した。
A 36-year-old female presented with unilateral lipid keratopathy of 4 months' duration. She was treated by lamellar keratoplasty. Light microscopy of the resected corneal stroma showed disorders in the collagenous lamellae, inflatration of inflamma-tory cells and vascularization. Stains for fatshowed neutral fat in the corneal stroma. particu-larly in the cytoplasm. Electron microscopy showed histiocyte cytoplasms containing numerous lipid globules. It appeared that lipid keratopathy was secondary to vascularization in asymptomatic cor-neal inflammation.
