We observed neovascular glaucoma 6 to 10 months after cataract surgery in 5 eyes of 4 diabetic patients. Four eyes had undergone extracapsular cataract extraction with intraocular lens implantation. One eye had undergone pars plana lensectomy. Systemic vascular complication had been present in 3 cases including internal carotid artery stenosis in 2 cases. In one case who underwent cataract surgery in both eyes,neovascular glaucoma developed in only one eye with ruptured posterior capsule. Neovascular glaucoma seemed to have been induced not only by diabetic retinal microangiopathy but also by systemic macro-angiopathy including internal carotid artery stenosis. The presence of intact posterior capsule appeared to suppress neovascular glaucoma.
