建築用ホッチキス（釘打機）による穿孔性眼外傷の2症例

Two cases of perforating eye injury due to industrial pneumatic stapler

　建築用ホッチキス（釘打機）による眼球穿孔外傷の2症例に対して，早期に硝子体手術を併用して良好な視力を保つことができたので報告する。眼球内容の脱出を最小限にするよう術中に穿孔創を縫合しながら摘出した。硝子体切除により硝子体出血，硝子体中の水晶体物質を除去し，網膜にかかる牽引の解除を計った結果，硝子体膜形成や牽引性網膜剥離を予防できたと思われた。今回使用された釘打機は共に安全装置がなく，また両症例とも保護メガネを着用していなかった。近年，釘打機は建築現場などで常用されており，今後労務災害防止のためにも釘打機の安全装置の付加や衝撃に強いポリカーボネイト製の保護メガネ着用などの対策が必要である。

Early vitrectomy for perforating injury caused by pneumatic stapler led to good and useful vision in 2 cases. The success was mainly due to removal of traumatized lens material and vitreous blood which prevented the formation of vitreous membrane and traction retinal detachment. Safety trigger lock would provide protection to pneumatic stapler workers. Polycarbonate material is advocated for spectacle lenses to protect the eye of these high-risk workers.
