We evaluated the effect of peroral cyclosporine at the initial dosis of 5 mg/kg/day in preventing graft rejection in high-risk keratoplasty patients. Enrolled in the study were 17 cases who underwent keratoplasty : penetrating keratoplasty 11 eyes, keratoepithelioplasty 8 eyes and combined proce-dure 2 eyes. No endothelial or epithelial rejectiondeveloped except in one case as long as oral cyclosporine was continued. Endothelial rejection developed in 4 eyes after discontinuation of cyclosporine. Three of these 4 cases responded well to corticosteroid and/or oral cyclosporine treat-ments. Graft survival rate was 73% in penetrating keratoplasty after a mean follow-up of 14.8 months and was 100% in keratoepithelioplasty after 10.8 months. We advocate oral cyclosporine in prevent-ing allograft reaction and in improving the visual outcome in high-risk corneal transplants.
