特異な経過をたどった周辺部角膜浸潤の1例

A case of unusual corneal marginal infiltrates

　上皮欠損を伴う原因不明の再発性周辺部角膜浸潤の1症例42歳女性を経験した。症例の角膜浸潤は副腎皮質ステロイド薬に反応して寛解し，増悪期には，口腔内アフタや皮膚の毛嚢炎様皮疹，虹彩毛様体炎など，診断基準を満たさないもののベーチェット病を疑わせる所見が同期して認められた。検査所見では増悪期の好中球遊走能亢進，組織抗原適合検査HLA-DR52陽性が認められた。この角膜浸潤の成因としてベーチェット病に類似した血管炎が関与していると考えられた。

A 44-year-old female presented with recurrent corneal marginal infiltrates of unidenified origin. She also manifested recurrent aphthous stomatitis, folliculitis and iridocyclitis. During exacerbations, she showed increased neutrotactic activity of polymorphonuclear leukocytes. She was positivefor HLA DR-52. The infiltrates resolved after treatment with topical and systemic corticosteroid. Above findings suggested that the corneal infil-trates were secondary to systemic vasculitis as a feature of Behçet's disease.
