A 72-year-old male underwent cataract surgery with intraocular lens implantation in his right eye 9 years before. He received penetrating keratoplasty for bullous keratopathy 17 months before. When seen by us, he was diagnosed as endophthalmitis with corneal involvement. Culture for causative microorganism from corneal scrapings was negative. Topical vancomycin followed by vitrectomy led to clinical cure with improved visual acuity. Another 67-year-old male received trabeculectomy in his right eye 5 years before. Penetrating keratoplasty was performed for bullous keratopathy 22 months later. He was referred to us for presumed fungus keratitis of 4 months' duration. Topical vancomycin induced clinical cure. These cases illustrate that topical vancomycin may be effective for intractable infectious keratitis which failed to respond to previous broad-spectrum antibiotics and corticosteroids.
