角膜移植・眼内レンズ挿入術後に難治性眼内炎を発症した2症例

Two cases of refractory endophthalmitis following penetrating keratoplasty and intraocular lens implantation

　72歳男性が右眼水晶体摘出術と眼内レンズ移植術を9年前に受けた。続発した水疱性角膜症に対して全層角膜移植が17か月前に行われた。角膜炎を伴う眼内炎が今回診断された。角膜擦過標本の培養は陰性であった。バンコマイシンの点眼と結膜下注射と，これに続く硝子体手術で臨床的治癒が得られた。67歳男性が右眼線維柱帯切除術を5年前に受け，続発した水疱性角膜症に対して全層角膜移植および白内障手術がその22か月後に行われた。真菌性角膜炎が4か月前に発症し，当科を受診した。バンコマイシン点眼で角膜炎は治癒した。今回の2症例は，広域抗生物質とステロイド薬が長期にわたって使用された後の難治性の角膜感染症に対してバンコマイシンの局所療法が有効であることを示す事例である。

A 72-year-old male underwent cataract surgery with intraocular lens implantation in his right eye 9 years before. He received penetrating keratoplasty for bullous keratopathy 17 months before. When seen by us, he was diagnosed as endophthalmitis with corneal involvement. Culture for causative microorganism from corneal scrapings was negative. Topical vancomycin followed by vitrectomy led to clinical cure with improved visual acuity. Another 67-year-old male received trabeculectomy in his right eye 5 years before. Penetrating keratoplasty was performed for bullous keratopathy 22 months later. He was referred to us for presumed fungus keratitis of 4 months' duration. Topical vancomycin induced clinical cure. These cases illustrate that topical vancomycin may be effective for intractable infectious keratitis which failed to respond to previous broad-spectrum antibiotics and corticosteroids.
