変視症を訴える白内障患者に対する白内障と硝子体の同時手術

Combined cataract extraction and pars plana peeling of epiretinal membrane

　両眼の老人性白内障と左眼の黄斑部網膜上膜形成による視力低下および変視症を訴える症例に対して，右眼に白内障手術，左眼に白内障手術と同時に硝子体手術を施行した。左眼白内障は硝子体手術が可能な程度の後嚢混濁であったが，黄斑機能の改善を言的に早期に網膜増殖膜の除去と白内障手術を行った。その結果，網膜機能の回復は順調で，視力は改善し，変視症も消失した。また，両眼視機能も良好となった。高齢者の増加に伴って白内障と黄斑病変を有する症例に遭遇することが多くなる。黄斑上膜の手術予後は良好なことから，白内障との同時手術は視機能の回復のために試みる方法と考える。

A 75-year-old woman presented with bilateral blurring of vision and metamorphopsia in the left eye. We identified senile cataract and epiretinal membrane in the left macula as causes of her complaints. She was treated by cataract extraction in the right eye and by combined cataract extrac-tion and pars plana peeling of the epiretinal mem-brane with additional photocoagulation. The surgi-cal outcome was gratifying. Visual acuity improved and the metamorphopsia disappeared. Cataract extraction combined by pars plana peeling of epir-etinal membrane is recommended as a safe and effective procedure.
