全層角膜移植術後シクロスポリン使用中に拒絶反応を呈した1症例

Graft rejection developed in a case of penetrating keratoplasty during systemic Cyclosporine therapy

　先天性遺伝性角膜内皮変性症の23歳女性に対し，右眼2回言（両眼延べ4回目）の全層角膜移植術を施行した。術翌日よりシクロスポリンを5mg／kgで内服治療を行った，術後1週に内服量を3mg／kgに減量し，その3週後の血中シクロスポリン濃度は免疫抑制有効範囲にあると考えられる250ng／mlであったが，その1週後に拒絶反応を生じ，角膜内皮機能不全状態となった。シクロスポリン内服による全層角膜移植術後の拒絶反応抑制効果については，過去の使用例での臨床経過の検討および症例の蓄積によって，その使用法，特に有効血中濃度の設定を中心に，さらに検討を行う必要があると考えられた。

A 23-year- old woman underwent repeated pene-trating keratoplasty for congenital hereditary en-dothelial dystrophy. After the second surgery for the right eye, or the 4 th for both, we administeredsystemic Cyclosporine at the daily dosis of 5 mg per kg of body weight. The dosis was reduced to 3 mg/ kg one week later. Four weeks after surgery, the plasma cyclosporine was at an effective im-munosuppressive level of 250 ng/ml. Acute rejec-tion developed one week later resulting in dysfunc-tion of corneal endothelium. It appeared that the dosis of Cyclosporine was not adequate in this particular case.
