Major tranquilizerによると思われるOculogyric crises症例について

A Case of Oculogyric Crisis Caused by Major tranquilizer (Toresten:Thiethylperazine)

緒言

　oculogyric crisesとは，発作的に出現する両眼の不随意的なけいれん性上方偏位であり，数分〜数時間の発作がみられ，oculogyric spasms，oculogyric seizures,visual fits,tonic eye fitsとも呼ばれる^15）。このoculogyric crisesは多くの場合，postencephalitic syndromeの1つとしてみとめられ，Onuaguluchi^23）によると29.8％の高率に出現するという。

　一方，1950年代の前半頃からmajor tranquil—izerとして精神科領域で多く用いられて来たphenothiazine誘導体（chlorpromazineその他）は，その副作用として錐体外路反応をきたし，老人よりも小児に頻々にみられることが報告せられている^11）。しかし，phenothiazine誘導体は，小児の悪心，嘔吐に対しては，その原因の如何にかかわらず有効であるために，日常繁用せられている薬物である^12）。major tranquilizerの副作用としてのoculogyric crisesの発症は，かなり大量，かつ長期にわたつて用いた場合にみられることが多い^28）。

In the present paper, the author reported acase of oculogyric crisis with retraction of the neck and torticollis in 14-year-old girl, which was caused by a small dosis of Toresten (1 tablet 6. 5 mg, less than 3 tablets).

This girl complained of severe nausea and vomiting with slight fever on December 8, 1972, and was diagnosed as the coryza. Tores-ten was prescribed for symptomatic relief of nausea and vomiting. Several hours after the oral administration of Toresten, an attack of oculogyric crisis was observed which lasted several hours.
