A 56-year-old male with intractable open angle glaucoma showed intraocular pressure of 30 mmHg in the right eye and 32 mmHg in the left. He refused filtering surgery. Argon laser trabeculoplasty and maximal medications failed to lower the intraocular pressure. One eye was then treated by transscleral cyclophotocoagulation with free-run-ning mode Nd: YAG laser at the power setting of5-6 J, 6 shots per quadrant. After applying 10 shots, the patient noted sudden visual loss. Retinal vessels and the optic disc could not been seen by fluores-cein angiography. Immediate systemic corticoster-oid and stellate ganglion block resulted in gradual improvement of the visual symptoms. We advocate that cyclophotocoagulation be limited to cases that fail to respond to other therapeutic modalities.
