脊髄腫瘍により視力低下を伴ううっ血乳頭を呈した1例

Papilledema and blurring of vision in a case of spinal cord tumor

　62歳男性が1か月前からの視力低下を主訴として受診し，うっ血乳頭が発見された。CTとMRIで頭蓋内腫瘍は否定された。髄液検査で脊髄圧と蛋白の上昇があった。脳室腹腔短絡手術後うっ血乳頭は軽快したが視力には大きな改善はなかった。術後，L1〜2とL5〜S1の位置に脊髄腫瘍が発見され，全摘された。組織学的には神経線維腫であった。

A 62-year-old male presented with blurring of vision since 1 month before. Funduscopy showed bilateral papilledema. Other neurological findings were normal. No intracranial tumor was detected by computed tomography or magnetic resonance imaging. Lumber puncture showed elevated pres-sure and increased protein content in the cere-brospinal fluid. Surgery shunting the cerebral venti-cles to the peritoneum resulted in resolution of papilledema without improvements in visual acu-ity. Two masses of spinal tumor were detected later at the level of L1-2 and L5-S1. Surgery showed the tumor to be neurofibroma.
