Abstract Purpose:We experienced a rare case of orbital apex syndrome caused by an extramedullary mass in the orbital apex during the course of acute myeloid leukemia(AML).

Case:A woman in her 70s presented with blurred vision, ocular pain, and oculomotor palsy in the right eye. She had AML and rheumatoid arthritis. MRI showed normal findings in the right orbit. During the course of the disease, worsening in visual acuity, decreased right critical fusion frequency, ptosis, and ocular motility disorder in all directions appeared in the right eye. PET/CT showed hyperaccumulation in the right orbital apex, which was diagnosed as orbital apex syndrome caused by an extramedullary mass of acute myeloid leukemia. Considering her general condition, steroid half-pulse therapy was performed. Blurred vision and pain in the right eye disappeared, and improved critical fusion frequency was obtained after the therapy. No recurrence was observed at two months post-treatment, when follow-up was terminated.

Conclusions:Although it is rare, we should be aware that extramedullary tumors could be a cause of orbital apex syndrome in patients with AML. If infection and other risks are ruled out, steroid half-pulse therapy may be effective in orbital apex syndrome caused by extramedullary tumors.
