Abstract Purpose:We report a case in which pupillary changes were recorded on the ORTe EYENAC(EYENAC, Japan Focus)during a 1% apraclonidine eye drop test.

Case:An 87-year-old woman presented to our hospital with decreased visual acuity in the right eye, and at the time of initial examination, pupillary discrepancy(left eye>right eye)was observed in the dark. At the time of initial examination, no obvious fundus findings were observed and the vision loss was thought to be owing to cataract. At the second visit, apraclonidine eye drops were administered, and pupillary movement was using by EYENAC before and after the apraclonidine eye drop test.

Results:The EYENAC showed a reversal of pupillary discrepancy. Mean while, there was no obvious abnormality in pupillary movement before and after the administration of apraclonidine eye drops.

 Apraclonidine eye drop test result was considered positive, and CT scan of the head and neck was performed. A neoplastic lesion was observed in the apex of the right lung, which was considered to be a schwannoma on imaging.

Conclusion:EYENAC was useful in recording and quantitatively evaluating the findings before and after apraclonidine eye drop administration in Horner's syndrome.
