Abstract. Purpose:To report a case of scleritis following phacoemulsification and transscleral fixation of intraocular lens(IOL). Case:A 71-year-old female with diabetes received cataract surgery by phacoemulsification and transscleral IOL fixation. Postoperative course was uneventful with visual acuity improving to 0.7. Two years later, the right eye started to show repeated episodes of pain, hyperemia and subconjunctival hemorrhage. The condition was diagnosed with scleritis. Topical corticosteroid was followed by improved symptoms. Ten months later, scleritis exacerbated and was associated with uveitis. Pulsed prednisolone therapy was performed under the diagnosis of necrotizing scleritis. The ocular inflammation gradually improved. Conclusion:Cataract surgery with transscleral IOL fixation may be followed by noninfectious necrotizing scleritis. Pulsed corticosteroid is one the therapeutic options.
