Abstract Purpose:To report a case of optic disc swelling and serous retinal detachment during administration of nivolumab.

Case:The patient was a 74-year-old man. He noticed distorted vision in the left eye and visited our department. He had been treated with nivolumab for 9 months for malignant esophageal melanoma(multiple liver and lung metastases). Visual acuity on initial examination was 1.2 in the right eye and 0.5 in the left eye, and the left eye showed marked optic disc swelling and serous retinal detachment(SRD)extending from the optic disc to the macula. He also had an SRD in the right eye, but it was very short and barely detectable on optical coherence tomography. Fluorescent fundus angiography showed strong fluorescence leakage from the optic disc in both eyes, especially in the left eye. We assumed that uveitis caused by hyperimmune reaction to nivolumab, and left intravitreal injection of triamcinolone acetonide was performed, which improved optic disc swelling and SRD in the left eye, and visual acuity in the left eye improved to 0.8 after 3 months.

Conclusion:Nivolumab can cause intraocular inflammation while having a high tumor suppressive effect. Local administration of corticosteroids was effective against intraocular inflammation.
