Abstract Purpose:To report a case of choroidal osteoma with serous retinal detachment(SRD)that responded to intravitreal injection of anti-vascular endothelial growth factor(VEGF).

Case:An 18-year-old woman was referred to us in January 2017 for visual failure, SRD in the right eye and retinochoroidal degeneration in both eyes. Best-corrected visual acuity(BCVA)was 1.0 in the right eye and 1.5 in the left eye. SRD was observed only in the right eye, and computed tomography revealed high-density areas in the right eye. Fundus examination and optical coherence tomography revealed choroidal lesions in both eyes;she was diagnosed with bilateral choroidal osteoma. Her BCVA in the right eye dropped to 0.7 after initial follow-up;however, follow-up continued because of widespread choroidal lesions including fovea in her right eye;no visual loss existed in her left eye. In January 2020, SRD was observed in the left eye. Although her BCVA of the left eye remained at 1.5, she complained of a dark spot in the center of her vision. SRD in both eyes was considered to be associated with choroidal neovascularization. The patient was treated with intravitreal injection of bevacizumab in both eyes. After treatment, SRD decreased in the left eye but not in the right eye. We additionally treated with two intravitreal injections of bevacizumab and one intravitreal injection of aflibercept in both eyes, however, without further improvement.

Conclusion:Intravitreal anti-VEGF injections were effective for SRD secondary to choroidal osteoma.
