Abstract Purpose:To determine the effect of ripasudil hydrochloride hydrate eye drop(ripasudil)on decreasing intraocular pressure(IOP)and inducing changes in Schlemm canal(SC).

Subjects and Methods:Twenty-two glaucoma patients were included in this study and received 6 weeks or more of ripasudil instillations in one eye from February 2020 to March 2021. We measured IOP and acquired SC temporal and nasal sections of AS-FD-OCT images of both eyes before and after ripasudil instillations. We calculated the SC length(SCL)and cross-sectional area(SCA)with ImageJ.

Results:Ripasudil was discontinued in three patients because of side effects. IOP of 19 eyes decreased significantly after ripasudil treatment from 17.7±4.5 mmHg to 16.4±4.6 mmHg(p<0.05). However, in the contralateral eye without ripasudil treatment, there were no significant changes. Although for SCL and SCA, there were no significant changes with both groups when compared before and after ripasudil treatment. In the ripasudil group, the IOP 10 eyes decreased by 2 mmHg or more(effective group), while that in 9 eyes did not decrease(no effective group). The SCL and SCA did not show any significant changes in both groups.

Conclusion:Ripasudil treatment significantly reduced IOP, but without inducing any change in SC size.
