Abstract Purpose:To report a case of severe cornel lesion following surgery in prone position under general anesthesia.

Case:A 50-year-old female received surgery to remove the tumorof the third thoracic spinal cord under general anesthesia. Surgery was performed in the prone position and lasted 4 hour and a half. The cornea was protected by a protection tape during surgery. The left eye developed pain and heperemia from the day of surgery. She was referred to us the following day.

Finding and Clinical Course:The left eye showed corneal epithelial erosion and conjunctival hyperemia. The corneal erosion healed 15 days after treatment with eyedrops. Ocular pain and hyperemia recurred 50 days after onset. The left eye showed corneal edema and hyperemia. Corneal endothelial count was 1,193 cells/mm2. Corneal edema gradually improved after treatment with eyedrops and soft contact lens. Visual acuity improved to 0.9 310 days after onset. Corneal cell density remained at 598 cells/mm2.

Conclusion:Corneal injury in the present case was suspected to be due to improper use of topical disinfectant containing chlorhexidine gluconate and 83 vol% ethyl alcohol.
