Abstract Purpose:To report a case of sympathetic ophthalmia that developed following scleromalacia after surgery for pterygium.

Case and Findings:A 78-year-old male received surgery for pterygium in both eyes. Scleromalacia developed in both eyes 3 months later and was treated by topical corticosteroid. Both eyes developed granulomatous uveitis 15 months after initial surgery. He received cataract surgery in both eyes 5 months after onset of uveitis. Both eyes showed extensive depigmentation in the choroid. Fluorescein angiography showed hyperfluorescence of the disc and window defects in both eyes. He later manifested depigmentation of the hair and skin. He was positive for HLA-DR4 and was diagnosed with sympathetic ophthalmia.

Conclusion:The present case illustrates that sympathetic ophthalmia may show scleromalacia as the initial manifestation.
