Abstract Purpose: To report a case who developed severe eyeglobe rupture after blunt trauma 20 years after radial keratotomy.

Case: A 41-year-old male fell down and was injured in the right eye. He had received radial keratotomy 20 years before.

Findings and Clinical Course: Visual acuity was light perception in the right eye and 1.0 in the left. The right eye showed 8 surgical wounds due to radial keratotomy. Three wounds, located at 2, 4 and 8 o'clock position, joined together at the corneal apex. The vitreous body relapsed through one of them. The anterior chamber and the lens were absent. The left eye showed similar wounds of radial keratotomy. Incision into the cornea occupied 90% of the corneal thickness. The density of corneal endothelial cells was 2,662/mm2. Final visual acuity in the right eye was light perception due to persistent vitreous hemorrhage.

Conclusion: The present case illustrates that surgical wound due to radial keratotomy may rupture following minor trauma even after several decades.
