Abstract Purpose:To report a case who received cataract surgery with temporary discontinuation of peroral imatinib mesylate to avoid conjunctival edema.

Case:A 67-year-old woman presented with impaired vision in both eyes. Best-corrected visual acuity(BCVA)was 0.9 in the right eye and 1.0 in the left. Both eyes showed idiopathic choroidal folds. Two years later, she developed stromal tumor of the rectum. It was surgically removed followed by peroral imatinib mesylate as adjuvant therapy. Conjunctival edema developed after two months of imatinib mesylate. Another one year later, BCVA decreased to 0.5 right and 0.8 left due to cataract. Surgery was recommended.

Result:In order to avoid conjunctival edema, imatinib was discontinued for one week before and after cataract surgery. The left eye was operated first, followed by surgery of the right eye with similar procedure. Visual acuity improved to 1.0 in either eye. Conjunctival edema recurred after peroral imatinib mesylate was resumed after surgery.

Conclusion:Cataract surgery was safely performed while temporarily discontinuing peroral imatinib mesylate to avoid conjunctival edema.
