Abstract. Purpose:To report a case of posttraumatic hypotony and maculopathy treated by vitrectomy and suturing of intraocular lens(IOL). Case:A 23-year-old male visited us after manually hit on his right eye. Right visual acuity was 0.1 when corrected by -12 diopters. The right eye showed hyphema, vitreous prolapse into the anterior chamber, mydriasis, subluxation of the lens, and cyclodialysis over one quarter of the circumference. Extreme hypotony with macular edema with visual acuity down to 0.09. Ten weeks after injury, the right eye received vitrectomy, lensectomy and suturing of IOL. Intraocular pressure improved to 16 mmHg 6 weeks later with visual acuity of 0.4 when corrected by -5.5 diopters. He has been doing well without macular edema for 11 months after injury. Conclusion:Vitrectomy with IOL suturing was effective for posttraumatic hypotony and maculopathy.
