Abstract. Purpose:To report a case of small cell lung cancer with bilateral central retinal artery occlusion(CRAO)as the initial manifestation. Case:A 77-year-old female noticed blurring of vision in the right eye and sought medical advice the same day. She had been a smoker in the past 18 years. Findings and Clinical Course:Corrected visual acuity was counting fingers right and 0.4 left. Both eyes showed retrocorneal precipitates, vitreous opacity, and cherry-red macula. The right eye was diagnosed with CRAO and the left eye with branch retinal artery occlusion. The left eye developed CRAO 5 days later with visual acuity down to 0.01. She showed elevated level of carcinoembryonic antigen(CEA)that led to detection of tumor in the mediastinum and upper lung lobe. Biopsy led to the diagnosis of metastatic small cell lung cancer. Systemic studies showed multiple cerebral metastases and attenuated right internal carotid artery. Systemic chemotherapy was followed by disappearance of signs of intraocular inflammation. There has been no flare-up for 6 months until present. Conclusion:This case illustrates that bilateral CRAO may be secondary to extraocular malignancy.
