眼科受診を契機に診断に至った尿細管間質性腎炎・ぶどう膜炎症候群の1例

A case of tubulointerstitial nephritis and uveitis（TINU）syndrome diagnosed during ophthalmological examinations

症例：14歳，女児。全身症状として体重減少，ふらつきが出現し近医を受診した。採血で軽度の貧血と腎機能低下を指摘された。経過観察を指示されたが，症状に改善はみられなかった。2か月後，両眼の霧視が出現したため，眼科を受診し両眼のぶどう膜炎の診断となった。

所見と経過：両眼に汎ぶどう膜炎がみられた。血液検査および尿中のβ₂ミクログロブリンの上昇がみられたため，間質性腎炎の合併があると判断した。これらの所見と臨床症状を考慮して，TINU症候群と診断された。ぶどう膜炎は副腎皮質ステロイド点眼で改善し，経過観察で腎障害の改善がみられた。

結論：TINU症候群では腎機能障害があっても軽微なことが多く，小児の場合は腎生検が施行されない場合も多い。その場合は，問診と検査をしたうえで臨床的診断が重要となる。全身症状を伴う原因不明のぶどう膜炎患者にはTINU症候群を念頭に置くことが重要である。

Abstract　Purpose：To report a case of tubulointerstitial nephritis and uveitis（TINU）syndrome diagnosed during ophthalmological examinations.

Case：A 14-year-old girl was referred to us for blurring of vision in both eyes. She complained of decreased body weight and dizziness 2 months before. She showed reportedly anemia and decreased kidney functions.

Findings and Clinical Course：Both eyes showed findings of panuveitis. Blood tests and increased level of β₂ microglobulin in the urine led to the diagnosis of interstitial nephritis and of TINU. Topical corticosteroid was followed by improvement in panuveitis. Interstitial nephritis showed spontaneous improvement.

Conclusion：This case illustrates that clinical manifestations may be slight and overlooked in the presence of kidney lesions in TINU. TINU syndrome ought to be kept in mind in idiopathic uveitis with systemic disorders.
