Abstract Purpose:Brolucizumab is a newly approved anti-vascular endothelial growth factor(VEGF)for age-related macular degeneration(AMD), which has been recognized to be effective at long dosing intervals of 12 weeks during the maintenance phase. We report a case in which brolucizumab monotherapy alone was effective for exudative AMD that recurred after photodynamic therapy(PDT)with ranibizumab.

Case:This report describes the case of a 71-year-old man. Since the subretinal fluid(SRF)did not improve even after 6 intravitreal injections of aflibercept at another hospital, he was referred to our department after PDT in January 2019. Two sessions of PDT with ranibizumab 0.5 mg were performed in December 2019 and May 2020. Since SRF relapsed despite these PDT sessions, intravitreal injections of brolucizumab 6 mg were administered on September and December in, 2020, and March, July, September in 2021.

Results:Improvements in SRF and pigment epithelial detachment were observed. No adverse events usually reported with brolucizumab, such as uveitis, retinal vasculitis, and retinal ischemic syndrome, were observed.

Conclusion:In recent years, PDT is often combined with anti-VEGF therapy. We experienced a case in which brolucizumab alone responded to exudative AMD in which SRF relapsed after two PDT sessions with 0.5 mg ranibizumab therapies. Brolucizumab monotherapy may also be effective for exudative AMD in patients after multiple PDT with anti-VEGF.
