Abstract Purpose:Linezolid LZD is an effective treatment for multidrug-resistant tuberculosis(TB). However, long-term use of LZD has been reported to cause peripheral neuropathy and optic neuropathy. We report a case in which visual acuity and visual field disturbances were observed 10 months after initiating LZD, and both visual acuity and visual field disturbances improved within 5 months after its discontinuation.

Case:A 33-year-old woman was diagnosed with tuberculosis in July 2022 and started on multidrug chemotherapy, including ethambutol. In October 2022, the multidrug-resistant TB was identified and the patient began receiving multidrug chemotherapy including LZD. Ten months after LZD initiation, the patient visited to our hospital with visual impairment. At the initial visit, the corrected visual acuity was 0.07 in the right eye and 0.08 in the left eye, with marginal flicker values of 22.5 Hz in the right and 33.0 Hz in the left. Both direct and indirect reflexes were positive, and relative afferent pupillary defects were absent in both eyes. A Goldmann dynamic visual field test revealed central dark spot, and LZD was discontinued because of suspected toxic optic neuropathy. Fourteen days later, at the return visit, corrected visual acuity was 0.06 in the right eye and 0.4 in the left eye, CFF was 14 Hz and 28 Hz, respectively, and a color vision test showed no abnormality. Five months after discontinuation of LZD, the corrected visual acuity improved to 1.2 in the right eye and 1.2 in the left eye, and the central dark spot was 1.2 on the right. The dark central spots had resolved.

Conclusion:Rapid improvements in visual acuity and visual field were achieved after discontinuation of LZD. When using linezolid, collaboration with the primary department before initiation and prompt intervention when adverse effects arise are essential.
