尿細管間質性腎炎・ぶどう膜炎症候群が疑われた中高年女性の1例

Tubulointerstitial nephritis and uveitis syndrome in a middle-aged female

要約　目的：尿細管間質性腎炎・ぶどう膜炎（TINU）症候群が疑われた中高年女性の1症例の報告。

症例：58歳の女性が両眼結膜充血を主訴に受診した。

所見：両眼の著明な結膜毛様充血と角膜後面沈着物があり，前房に炎症細胞がみられた。副腎皮質ステロイドの点眼および内服で改善しなかった。尿中β₂ミクログロブリン（β₂-MG），N-アセチルグルコサミニダーゼ（NAG）の上昇が確認され，TINU症候群が疑われた。ステロイドパルス治療により炎症は徐々に軽快した。

結論：原因不明の両眼性前眼部ぶどう膜炎では，中高年であってもTINU症候群を考慮して，尿中β₂-MGとNAGの検索を行うことが必要である。

Abstract　Purpose: To report tubulointerstitial nephritis and uveitis（TINU）syndrome in a middle-aged female.

Case: A 58-year-old female presented with conjunctival hyperemia.

Findings: Both eyes showed severe ciliary injection, keratic precipitates and cells in the anterior chamber. Topical and systemic corticosteroids were not effective. Elevated levels of urinary β₂ microglobulin（β₂-MG）and N-acetylglucosaminidase（NAG）led to the diagnosis of TINU syndrome. Corticosteroid pulse therapy was followed by gradual improvement of anterior segment inflammation.

Conclusions: Bilateral idiopathic anterior uveitis in middle or old-aged patients may be caused by TINU syndrome. Urinalysis includingβ₂-MG and NAG may be useful in the diagnosis.
