良好な経過をたどった尿細管間質性腎炎・ぶどう膜炎症候群の2例

Two cases of tubulointerstitial nephritis and uveitis syndrome with fair outcome

要約　目的：尿細管間質性腎炎・ぶどう膜炎症候群（TINU）の2症例の報告。症例：55歳と21歳女性で，前者では両眼のぶどう膜炎，後者では全身の倦怠感で初発した。両者とも腎機能が増悪し，生検が行われた。その結果，間質性腎炎と診断され，眼症状とあわせてTINU症候群の診断が確定した。副腎皮質ステロイドの点眼と内服で，前眼部が主体のぶどう膜炎と腎機能障害を含む全身症状は速やかに改善した。結論：TINU症候群では前部ぶどう膜炎が先行することがある。ぶどう膜炎は一般に予後が良好なので，後発する腎障害が見落とされる恐れがある。原因不明なぶどう膜炎では，本症を念頭におき定期的な血液検査が望まれる。

Abstract.　Purpose：To report two cases of tubulointerstitial nephritis and uveitis（TINU）syndrome. Cases：Both were females aged 55 and 21 years respectively. Initial manifestations were bilateral anterior uveitis in one case and fatigue with weight loss in the other. Renal function deteriorated in both cases. Biopsy showed tubulointerstitial nephritis,leading to the diagnosis of TINU syndrome in both cases. Topical and systemic corticosteroid was followed by prompt improvement of uveitis and systemic manifestations including impaired renal function. Conclusion：These cases illustrate that TINU syndrome is preceded by anterior uveitis that frequently responds to treatment. In order not to overlook subsequent TINU syndrome,we advocate repeated hematological examinations in cases of unclassified uveitis.
