要約 目的:視神経乳頭浮腫を伴った多発消失性白点症候群(MEWDS)に対しステロイド治療を行った1例の報告。
症例:35歳,女性。感冒様症状発症4日後に右眼視野障害を自覚し,当院を紹介され受診した
所見と経過:矯正視力は右0.7,左1.2。眼底検査では右眼後極部の多数の白斑,両眼視神経乳頭の発赤・腫脹を認めた。その後,右矯正視力が0.15と低下し,右眼視野の増悪を認めたため,ステロイド全身投与を開始した。投与開始5日後から視力は改善に転じ,投与開始15日後に右矯正視力1.0まで改善し,視野検査で暗点の改善,眼底の白斑の消失を認めた。
結論:視力低下が顕著なMEWDSに対し,早期のステロイド全身投与により病期を短縮できた可能性がある。
Abstract Purpose:To report a case of multiple evanescent white dot syndrome(MEWDS)with optic disc edema.
Case:A 35-year-old female presented with visual field defect in the right eye. She had suffered from presumed common cold 4 days before.
Findings and Clinical Course:Corrected visual acuity was 0.7 in the right eye and 1.2 in the left eye. The right eye showed numerous white dots in the posterior fundus. Both eyes showed edema and hyperemia of the disc. Visual acuity in the right eye decreased to 0.15 with central scotoma 4 days later. She then started receiving peroral prednisolone. Visual acuity showed signs of improvement 5 days later. Central scotoma and white dots in the fundus disappeared another 15 days later.
Conclusion:The present case suggests that peroral prednisolone may induce early resolution of fundus lesions and visual function associated with MEWDS.
