Abstract Background:Optic perineuritis is a clinical entity with inflammation located in the optic sheath.

Purpose:To report two cases of optic perineuritis who received pulsed corticosteroid therapy.

Cases:Both were females aged 31 and 54 years respectively. One had impaired vision with central scotoma in one eye since 6 months before. The other noted impaired vision in one eye and severe headache since one week before.

Findings and Clinical Course:Corrected visual acuity in the affected eye was 0.7 and 0.4 respectively. Magnetic resonance imaging(MRI)showed enhanced signal around the optic nerve in both cases. Following pulsed corticosteroid therapy, visual acuity improved in one case and headache disappeared in the other with unchanged visual acuity. Both cases showed recurrence of ocular lesion after tapering and discontinuation of prednisolone.

Conclusion:The present cases show that pulsed corticosteroid therapy may be effective for whole or part of signs of optic perineuritis. Early discontinuation of peroral corticosteroid may be followed by recurrence.
