特定健診導入後8年間の当院眼科健診結果の検討

Review of cases seen during 8 years after introduction of specific health check-up system for adults

要約　目的：過去8年間に当院を受診した眼科健診受診者の緑内障の早期発見とその他眼疾患の検出。

対象と方法：2008年4月〜2016年3月の間に国民健康保険加入者および長寿（後期高齢者）医療保険加入の世田谷区民で，受託医療機関から眼科健診を依頼された初診者1,755名（3,508眼）を対象とした。規定4項目（細隙灯顕微鏡検査，精密眼圧検査，精密眼底検査，眼底カメラ撮影）で異常を認めた全例に追加検査（視力検査，角膜厚検査，隅角検査，光干渉断層計，精密視野検査，広角カメラ撮影）を実施した。

結果：疾患別粗有病率は，緑内障5.75％，白内障61.2％，糖尿病網膜症7.7％，加齢黄斑変性1.1％，その他24.25％であった。

結論：緑内障については，多治見スタディの結果と同様の傾向を認めた。白内障については，80歳以上の高齢になる以前の白内障手術の奨励と，短期間での両眼白内障手術が必要である。糖尿病網膜症および加齢黄斑変性についても，発症増加が認められる。

Abstract　Purpose：To present a review of cases who were detected to have eye disorders, including glaucoma, during the past 8 years within the framework of specific health check-up system for adults.

Cases and Method：This study was made on 3,508 eyes of 1,755 persons who were referred to us during 8 years from April 2008 through March 2016. All were holders of National Health Insurance policy or latter-stage Elderly Healthcare insurance policy. All were initially screened by slit-lamp microscopy, tonography, funduscopy, and fundus photography. Those who failed in the screening process were tested for visual acuity, corneal thickness, gonioscopy, optical coherence tomography, perimetry, and wide-angle fundus photography.

Results：Cataract was present in 61.2％, diabetic retinopathy in 7.7％, glaucoma in 5.75％, age-related macular degeneration in 1.1％, and other eye disorders in 24.25％.

Conclusion：Incidence of glaucoma was similar to findings by the Tajimi Study. There was an increase regarding the onset of diabetic retinopathy and age-related macular degeneration.
