Abstract Purpose:To report a case of choroidal osteoma with choroidal neovascularization(CNV)as observed by optical coherence tomography angiography(OCTA).

Case:A 65-year-old female noted sudden central scotoma and metamorphopsia and visited us the same day.

Findings and Clinical Course:Corrected visual acuity was 1.5 in the right eye and 0.3 in the left eye. The left eye showed hemorrhagic detachment of retinal pigment epithelium(RPE), serous retinal detachment, and subretinal hemorrhage. Fluorescein and ICG angiography showed CNV. A yellowish choroidal lesion appeared after removal of subretinal hemorrhage by intravitreal injection of SF6. Serous retinal detachment, subretinal hemorrhage and hemorrhagic RPE detachment disappeared after three injections of aflibercept. Computed tomography(CT)showed calcification in the posterior wall of the left eye, leading to the diagnosis of choroidal osteoma. OCTA showed reduced blood flow in the choriocapillaris layer posterior to the choroidal osteoma.

Conclusion:Intravitreal injection of SF6 was followed by removal of subretinal hemorrhage and hemorrhagic RPE detachment. OCTA showed reduced blood flow in the choriocapillaris layer posterior to the choroidal osteoma.
