高知医療センターにおける未熟児網膜症の発症関連因子および治療適応因子の検討

Factors for pathogenesis and therapeutic outcome of retinopathy of prematurity at Kochi Health Science Center

要約　目的：高知医療センターにおける未熟児網膜症（ROP）の発症に関連する因子と治療適応となる因子の検索。

対象と方法：2013年1月〜2015年12月の3年間に眼底精査を施行した新生児158例をレトロスペクティブに調査した。

結果：ROPは47例（29.7％）に発症し，そのうち19例（12.0％）は光凝固治療を受けた。出生体重，在胎週数，動脈管開存，人工換気，濃厚赤血球輸血が発症に関連する因子であった（p＜0.01）。治療適応の判定に関与する因子は確認されなかった。光凝固を受けた全例が鎮静化した。

結論：高知医療センターにおいても，既報と同様にROPの発症には，出生体重，在胎週数，人工換気，動脈管開存，濃厚赤血球輸血が影響していた。

Abstract　Purpose：To report factors rerated to occurrence and therapeutic application of retinopathy of prematurity（ROP）in Kochi Health Sciences Center.

Cases：This retrospective study was conducted regarding 158 infants who received fundus examinations during 3 years from 2013 to 2015.

Results：ROP developed in 47 infants（29.7％）. Nineteen infants（12.0％）underwent laser photocoagulation treatment. Birthweight, gestational age, patent ductus arteriosus, artificial ventilation, and packed red blood cell transfusion were identified as significant risk factors for the development of ROP. Regression of ROP was attained in all treated cases.

Conclusion：Similar to previous reports, birthweight and gestational age, patent ductus arteriosus, artificial ventilation, and packed red blood cell transfusion played a role in the development of ROP in Kochi Health Sciences Center.
