未熟児網膜症の重症化因子の検討

Risk factors of retinopathy of prematurity

　東京女子医大母子総合医療センターにおいて最近6年間に管理された低出生体重児のうち，未熟児網膜症がⅢ期以上に進行した症例について検討した。厚生省分類でⅢ初期となった40例とⅢ中期となった40例を2群に分けて検討してみると，ROPの進行中には出生体重，在胎週数，輸血回数，気管支肺異形成，高頻度人工換気装置の使用，ROP進行期の体重増加率が関与していると考えられた。人工換気日数，呼吸窮迫症候群，酸素投与期間とROPの進行の間には関係はみられなかった。

　瘢痕を生じた症例は80例中39例であったが，このうち視力に影響のあるⅡ度以上のものは20例であった。呼吸管理が向上してからは瘢痕Ⅱ度以上の症例は減少している。

We tried to identify the factors for severeretinopathy of prematurity (ROP) through areview of 529 cases during the foregoing 6-yearperiod. The series included 80 low-birthweightbabies of less than 2,500 g who manifested stage 3of ROP. These were again divided into 2 groups:40cases with mild stage 3 of ROP and 40 cases withsevere stage 3 of ROP.We compared these twogroups and found, as contributing factors for sever-ity of ROP, birthweight, gestational age, bron-chopulmonary dysfunction, respiratory distress syn-drome, use of high-frequency oscillation, bloodtransfusion, and rate of gain in body weight. Sever-ity of ROP was not associated with duration ofartificial respiration or duration of oxygen therapy.
