未熟児網膜症手術治療例の遠隔成績

A follow-up study of retinopathy of prematurity, with special reference to the visual functions of the eyes treated by photocoagulation and/or cryocautcry

　光凝固，冷凍凝固および両者併用のいずれかの手術治療をうけた未熟児網膜症患者22例35眼の瘢痕期病変を観察し，可能なかぎり視機能の評価を試みた。観察期間は術後最短1年，最長9年半であった。

　術後眼底所見のうち，凝固瘢痕のみ，または凝固瘢痕＋瘢痕期I度（鳥大変法，以下同様）の"著効"は35眼中25眼の71.5％，凝固瘢痕＋瘢痕期II度の"軽快"は35眼中6眼の17.2％に認められた。

　視力に関しては，瘢痕期I度までであれば比較的良好な視力が得られたが，屈折異常の点では瘢痕期病変だけでは整理がつかない結果となった。

　眼位は27治療眼中24眼の88.9％，固視は17治療眼中14眼の82.4％，視野は11治療眼中6眼の54.5％に正常であったが，視野に関しては必ずしも正確な情報が得られたとはいえない。

　本症に対する手術療法の視機能におよぼす影響は複雑で，なお長期の観察が必要である。

A total of 35 eyes in 22 cases of retinopathy of prematurity in the cicatricial phase after treatments with xenon arc photocoagulation and/or cryocautery were examined as to visual functions which included visual acuity, refractive errors, ocular position, visual fixation and visual field. Follow-up periods ranged from one year to 9 and halfyears.
