Abstract Purpose:Human herpesvirus 6(HHV-6) can cause uveitis in rare cases. This report presents a case of HHV-6-associated uveitis successfully treated with foscarnet and valganciclovir.

Case:A woman in her 80s presented with sudden onset of blurred vision in the left eye and floaters. The corrected visual acuity at the initial examination was 0.9 and 0.4 in the right and left eyes, respectively. Slit-lamp examination revealed no evidence of inflammation in the posterior cornea and anterior chamber of either eye;however, white exudate and vitreous opacity were observed in the left eye. Multiplex PCR of the aqueous humor and vitreous humor was positive for HHV-6 only, and blood tests failed to identify no other cause. Therefore, the patient was diagnosed with HHV-6-associated uveitis, and intravenous foscarnet and oral valganciclovir were started.

Results:Retinal exudate decreased over time following the administration of foscarnet and valganciclovir. Furthermore, visual function improved to a corrected visual acuity of 0.9 in the left eye on day 120. The retinal exudate persisted even after one year but continued to decrease.

Conclusion:Reactivation of HHV-6 can occur as a secondary reaction of the uveitis. However, it may be the primary cause when no other pathogen-derived genome is detected and the copy number of HHV6-DNA is high, as observed in this case. Treatment with antiviral drugs exhibiting anti-HHV-6 activity is effective.
